Please go to the following website and answer the questions (if you are uncomfortable answering the questions about your own health information, you may answer the questions for a hypothetical person). You do not need to give an email address at the end. Just close the email request window, and it will show your results.
When you have completed the survey, please write about how you felt about the survey and results. Were you surprised by the results? Do you think they are accurate? How do you think the answers to questions about health behaviors influence your results? You can even play around a bit with the calculator to see how the results change when the answers to those questions change. Your original post is due Wednesday 11:59 p.m., and your response post is due Sunday at 11:59 p.m.
