For this Learning Activity, I would like you to think about your final wishes as if you are nearing the end of your life.  Write out an End of Life Plan with the following elements:

Health Care POA/Advanced Directive:

Who would you like to make your medical decisions in the event you are unable to make them for yourself?

How do you feel about Euthanasia?

Would you consider a DNR?

Where do you want to live your final days and/or die?

Will:

Who would you like to manage your estate in the event of your passing?

How would you like your assets distributed?  

Who would be your beneficiaries?

Are there any charities you would like to support?

Funeral:

Would you like to be buried or cremated, or some other means?

What special religious or cultural instructions would you ask of your family?

What type of funeral would you like?

Who would you like to speak at your funeral?

Do you have any special instructions or wishes?

How do you want to be remembered?

What final lessons or messages would you want to leave for your loved ones?
