CASE STUDY
You are the RN caring for a 34-year-old woman newly diagnosed with type 2 diabetes. She has a history of multiple sclerosis for 8 years and has been relatively stable for the last 7 years on interferon (IFN) β-1b injections. She also has a history of primary hypothyroidism (secondary to congenital absence of thyroid), hyperlipidemia, peripheral neuropathy, and nephropathy, but no evidence of hypertension or coronary artery disease. Her provider is considering treatment with Lantus and regular insulin, chlorpropamide (Diabinese), glyburide (Micronase), and metformin (Glucophage). Her 10-year-old daughter is also recently diagnosed with diabetes.

Diabetes Mellitus and Treatment
Explain the pathophysiology (the disease and changes to the cell, tissue, organs, and body functions) of diabetes mellitus and include the alterations in metabolic pathways and changes to basement membranes that occur. When you explain the disease process, explain where and how Lantus and regular insulin, chlorpropamide, glyburide, and metformin mechanism of action is used to treat the disease.

Explain the lifespan considerations for children who use antidiabetic agents.

Medication Counseling Needs
Explain the key medication counseling needs for this patient/family before, during, and after receiving Lantus and regular insulin, chlorpropamide, glyburide, and metformin (including major side effects & when the patient/family should contact their provider). Also, describe what reputable website the patient/family may use to find more information about this disease and the treatment.

Many people think that diabetes is only caused by excessive sugar consumption. However, many risk factors result in this type of illness. These risk factors include weight, genes, sedentary lifestyle, race, high blood pressure, ethnicity, polycystic ovary syndrome, and others. Type II diabetes is a very common disease that causes an individual’s blood sugar to rise beyond the normal threshold. Based on studies, diabetes is also associated with different diseases such as the development of multiple sclerosis in type II diabetic individuals. The previous history of the woman includes hyperlipidemia which is a disease that is present with the patient who has diabetes. Another complication caused by diabetes is peripheral neuropathy and nephropathy. Due to untreated diabetes, the disease caused serious problems to the blood vessels. The patient is lucky enough to not have signs of hypertension as nephropathy could result in high blood pressure. On the other hand, the daughter of the patient is diagnosed early with diabetes which can be managed more effectively.
